An NDT report should do more than state pass or fail. A useful record connects the examination to the correct item, method, procedure, personnel, equipment and governing requirements so the work can be reconstructed later.
Identify the work
Capture the project and item, weld, joint, component or drawing reference needed to locate the examination. Clear identification is the foundation of traceability.
Identify the examination basis
Record the NDT method and the applicable written procedure or instruction. Depending on the method and governing requirements, additional technique, equipment, consumable, calibration or image-quality information may be required.
Identify personnel and equipment
Records should support verification that the examination was performed by appropriately qualified personnel using controlled equipment where required. The specific qualification and calibration requirements come from the governing documents and written practice.
Document indications and disposition
Record relevant observations or indications in a way that connects them to their location. Acceptance or rejection should be based on the applicable criteria, not criteria invented by the report form.
Maintain reinspection traceability
If repair or additional examination occurs, link the new report to the original record so the final status is clear.
